What Is Classified as Whiplash?
Whiplash is a soft tissue injury to the neck caused by a sudden rapid acceleration-deceleration movement — most commonly in a rear-end road accident, but also in contact sports, falls, and physical assault. The rapid forward-and-back motion of the head places the cervical muscles, ligaments, discs, and joints under forces they are not designed to absorb, causing varying degrees of strain and irritation. Whiplash is graded from 0 to 4 using the Quebec Task Force classification, with Grades 1–3 being the most common presentations seen in physiotherapy
Common Causes for whiplash
- Rear-end vehicle collisions, the most common cause, where the head snaps back then forward
- Front or side impact vehicle accidents
- Contact sports — rugby, football, martial arts
- Falls with sudden head movement
- Physical assault or shaking
Symptoms for whiplash
- Neck pain and stiffness often developing 12–24 hours after the injury rather than immediately
- Headache, typically starting at the base of the skull and spreading forward
- Reduced neck movement, difficulty turning the head to one or both sides
- Shoulder and upper back pain
- Dizziness or balance disturbance may indicate upper cervical joint involvement
- Arm pain, numbness, or tingling suggests nerve root involvement
- Jaw pain from TMJ involvement in some accidents
