Patient Paperwork

This page has everything you need to prepare for your first visit

What to Expect

Your First Appointment, Step-by-Step

You’ll complete a short intake form, followed by a one-to-one consultation and physical assessment with your physiotherapist.

1. Initial Physiotherapy Assessment

Your first appointment begins with a discussion about your health history, symptoms, daily activities and recovery goals. Your physiotherapist will then assess your movement, strength, flexibility and the area causing concern. If further medical investigation may be useful, we will explain the appropriate next step.

2. Assessment Findings and Goals

Your physiotherapist will explain the assessment findings in clear language, discuss factors that may be contributing to your symptoms and agree practical treatment goals with you.

3. Personalised Treatment Plan

We’ll create a treatment plan tailored to your condition, lifestyle and goals. Depending on your needs, this may include targeted exercises, movement retraining, manual therapy, education and a home exercise programme.

Prepare Before You Arrive

Complete the relevant patient forms before your appointment and bring them with you, together with any medical reports or imaging that relate to your condition.

If you intend to make an insurance claim, please check your policy directly with your insurer before your visit. Physio Friend does not provide direct insurance billing. Payment is made to the clinic, and we will issue an itemised invoice and receipt for you to submit to your insurer yourself.

Coverage, required documents and reimbursement amounts depend on your individual policy.

How Insurance Claims Work

Physio Friend does not have direct billing arrangements with insurance providers. Patients pay the clinic at the time of their appointment and submit any insurance claim themselves.

  1. Check with your insurer whether outpatient physiotherapy is covered under your policy.
  2. Pay for your appointment directly at the clinic.
  3. Receive an itemised invoice and receipt from us.
  4. Submit the documents to your insurer according to its claim process.

Approval and reimbursement are determined by your insurer and individual policy. We cannot verify benefits or guarantee that a claim will be approved.
